Leverandøren skal ha utført minst 2 bestillinger som består av design, utførelse og implementering av et IKT-system for elektronisk informasjonsutveksling og integrasjon med eksterne systemer, hver til en verdi av minst 5 000 000,00 PLN brutto, i løpet av de siste tre årene før tilbudsfristen, eller i løpet av den kortere perioden dersom virksomheten har vært drevet i kortere tid.
Documentation Requirement
Leverandøren skal levere en liste over utførte leveranser som bekrefter oppfyllelsen av vilkåret spesifisert i kapittel VIII, punkt 1 i SWZ, og i tilfelle periodiske eller kontinuerlige tjenester, også de som utføres, i løpet av de siste 3 årene før tilbudsfristen, eller i løpet av den kortere perioden dersom virksomheten har vært drevet i kortere tid, med angivelse av deres verdi, gjenstand, utførelsesdatoer og enheter som leveransene ble utført for, samt vedlegg av bevis som angir om disse leveransene ble utført eller utføres på en forsvarlig måte.
